INSTRUCTIONS ON FILLING IN THE ORDER FORM

1. Fill in the name you wish to be printed on your package

 

2. Fill in an active email address for me to send you the invoice

 

3. Fill in your phone number in case I have to contact you

 

4. Fill in the address you wish me to mail your package to

 

5. Fill in your mailing type by choosing (a, b or c) from the following options

    a. Normal Mail (+S$1.00)

    b. Registered Mailing (+S$3.50)

    c. Free Normal Mail (*only for orders over $20)

 

6. Fill in your payment type by choosing (d, e, f or g) from the following options

    d. Concealed Cash (at your own risk; money must be mailed out within 72 hrs)

    e. Cash Deposit (within 72 hrs)

    f. Bank Transfer (within 72 hrs)

    g. Internet Banking (within 48 hrs)

   

7. At the space which says "MAILING & PAYMENT TYPE", fill in your options in the following format:

    <a,b or c>_<d, e, f or g>

    e.g. if i choose normal mail and cash deposit, write this------> a_e

 

7. Fill in the orders you wish to make, in the "ORDERS" space, in the following format:

    <name of product>_<quantity>_<color>

    e.g. mandybow_1_maroon

 

8. Fill in the total price in the "ORDERS" space AFTER stating your orders

 

9. Fill in any valid discount codes (if you know of any) after stating the total price, also in the "ORDERS" space

 

10. Then, CLICK the "SEND" button!

 

11. Lastly, wait for your invoice to be sent to the email address you entered.
